Minimum Qualifications

Essential Criteria

• Registered Nurse (NSW) with current unencumbered AHPRA Registration

• Demonstrated experience in a busy health environment dealing with patients experiencing a range of health challenges

• Proven ability to manage competing demands and complex clinical cases

• Able to work independently as well as be part of a multi-disciplinary clinical team

• Able to demonstrate creativity and initiative in relation to patient care

 • Excellent communication, time management and organizational skills

• Demonstrated knowledge and understanding of, respect for and compliance with culturally appropriate ways of working with Aboriginal individuals, families and communities

• A high standard of report writing including data entry in a range of health systems

• Drivers’ License

About The Position

Key Duties

The primary duties of this position include, but are not limited to:

• Provision of high quality nursing assessment, planning, implementation, and evaluation using best practice principles as part of a collaborative partnership with clients, families carers and other key stakeholders

• Work independently and collaboratively in a multidisciplinary team utilising professional skills and ability to contribute to client centred care to meet the clients individual health care needs, with the aim of maintaining and/or improving their health

• Work with the Aboriginal community and with the broader community where required

• Liaise with Aboriginal clients and other staff and assist with follow up of client treatments

• Monitor local Aboriginal health problems and needs, advising the Team Leader Clinic and/or professional staff

• Assist General Practitioners as required

• Provide nursing assessment, care coordination, evidence based best practice clinical intervention, monitoring, maintenance and education to clients and their carers/families with the focus on the Aboriginal community

• Provide emotional support and comfort to clients and to advocate for and facilitate the involvement of the client and their carers/families in planning and evaluating care

• Utilise skills, critical thinking and reflective practice to enhance decision making through reporting changes in the health and functional status of clients, including contributing to case discussions and case reviews

 • Utilise problem solving approach to clients centred care that includes the development of comprehensive care plans and the provision of case planning

• Communicate effectively with clients to provide an understanding of service delivery processes and clinical interventions, to ensure client’s needs are identified and met

• This position has responsibility for managing and monitoring cold chain within the clinic in accordance with RACGP Standard 5.3.2.B
